This questionnaire will go through all that is needed to make sure that all of the information is perfect for your menus! There is also a section at the bottom to add your on files if you have a print out of your menu or if there are specific ones that need to be kept separate due to dietary requirements. I look forward to seeing your menu choices and creating your menus for your wedding!
